It is with sincere gratitude and deep appreciation that I write to inform you that our office, Delaware Center of Excellence in Obstetrics and Gynecology, P.A., will be closing, effective July 1, 2026.

Please note: Access to the Athena patient portal will only be available through June 30, 2026.

We recommend downloading or sending your medical records before this deadline.
You can easily download or send your medical records through the patient portal by following the steps below: 

Step 1: Log into the Patient Portal     
•    Go to the patient portal and sign in.    
•    Click “My Health” on the left side.    
•    Select the “Health Records” tab.

‍Step 2: Choose Your Records     
•    Select the date range you need:    
◦    Custom range    
◦    Single date    
◦    Or “All time” for your full record
‍
‍Step 3: Choose What You’d Like to Do:     

•    To Download Your Records:    
◦    Click the “Download” tab.    
◦    Click Download.    
◦    Your records will be saved as a file on your computer.    

•    To Send Your Records by Email:    
◦    Click the “Email” tab.    
◦    Enter the recipient’s email address.    
◦    Confirm the email address.    
◦    Click Send.    

•    To Send Records Securely (Direct Messaging):    
◦    Click the “Direct Exchange” tab.    
◦    Enter the provider’s Direct Address (usually contains “@direct”).    
◦    Click Send.    
◦    If you’re unsure of the recipient’s Direct Address, you may need to contact their office directly.

Important: If you do not have access to the Athena patient portal and would like access, please call our office so we can update your email information.

If you would like a paper copy of your medical records, you must complete a Medical Records Release Consent form, available at our office. 
‍
We are processing medical record requests in the order that they are received. Once we receive consent forms we will call to inform you of any processing fees for printing (plus mailing after June 30, 2026) and let you know when your records will be available for pickup.

You will need a valid form of identification to pick up your medical records.
